We work with adults and children who have a learning disability and may need additional support to access our services.
What we can help with
We want to ensure you receive the best possible care and reasonable adjustments can be made to our services to meet your individual needs.
We can also co-ordinate care for when you attend hospital. In addition, we can make sure that all those involved in your care are aware of your additional needs.
We support and advise our staff on personalised care and delivery of services and can also act as facilitator between different organisations involved in your care.

The service is available Monday to Friday (excluding bank holidays) 8:00am – 4.30pm
