The team provide a wide range of intervention including:
- Equipment provision
- Falls prevention
- Mobility assessment
- Future planning conversations (preferred place of care and death)
- Home visits
- Symptom management (fatigue, breathlessness, anxiety)
- Bereavement support
- Onward referral (patients and carers)
- Signposting
Combined Supportive Care Service
The team also work alongside a Consultant in Palliative Medicine at St Oswald’s Hospice one day per week, providing care to lung cancer patients with a specialist palliative care and therapy need in the Combined Supportive Care Service (CSCS). In addition to the above LCOTS interventions, the CSCS promotes access to wider hospice outpatient services including:
- Complimentary therapy
- Group sessions (fatigue, breathlessness, relaxation, men’s/women’s drop-in)
- Social work
- Ambulatory care (blood transfusion, routine injections)
- Dietitian
- Psychology
- Medication changes/review
