The team provide a wide range of intervention including:
- Equipment provision
- Falls prevention
- Mobility assessment
- Future planning conversations (preferred place of care and death)
- Home visits
- Symptom management (fatigue, breathlessness, anxiety)
- Bereavement support
- Onward referral (patients and carers)
- Signposting
Combined supportive care service
The team also work alongside a consultant in palliative medicine at St Oswald’s Hospice one day per week, providing care to lung cancer patients with a specialist palliative care and therapy need in the combined supportive care service (CSCS).
In addition to the above lung cancer outpatient therapy service interventions, the Combined supportive care service promotes access to wider hospice outpatient services including:
- Complimentary therapy
- Group sessions (fatigue, breathlessness, relaxation, men’s/women’s drop-in)
- Social work
- Ambulatory care (blood transfusion, routine injections)
- Dietitian
- Psychology
- Medication changes/review
Both services work closely with community teams and the lung cancer multidisciplinary team to provide integrated, coordinated care and support.
