Contact
If you are caring for a patient who has undergone TPIAT and they have presented to your emergency department, or are an inpatient locally, and you require advice on their ongoing management, please contact the Freeman switchboard and ask to speak to the HPB registrar on call.
Referral criteria
There are inclusion and exclusion criteria for patients being considered for TPIAT.
Inclusion criteria
- TPIAT should be reserved for patients with intractable recurrent acute, chronic, or hereditary pancreatitis who have failed concerted prior treatment efforts to control recurrent pancreatitis or constant pain, because of the risk of ongoing post-operative pain and new onset diabetes.
Exclusion criteria
This excludes patients with specific medical conditions:
- C-peptide negative diabetes
- Type 1 diabetes
- Portal vein thrombosis
- Portal hypertension
- Significant parenchymal liver disease
- High-risk cardiorespiratory disease
- Pancreatic cancer
Patients with a poor support network may be considered a relative contraindication, as long-term management requires pancreatic enzyme supplementation and diabetes care. This will be assessed within the multidisciplinary team.
How to refer
If you are a patient, please discuss this with the local gastroenterology or surgical team currently managing your pancreatitis.
