This information is to help you (and your carers) about getting ready to move to the adult service. The adult home ventilation team is called the North East Assisted Ventilation Service (NEAVS).
What is transition?
Transition is the term used to describe the move from children’s to the adult services.
Transition starts around age 14.
The process
The move to adults is done in steps:
- From the age of 14 we will start to talk to you about moving to the adult service.
- By the age of 16, you will have some joint appointments with the adult team.
- You will be seen in children’s outpatients
- Your appointments will still be led by the children’s team

- You can ask to see the team on your own or with your parents/carers
- You may have more than one appointment
- You will be seen in children’s outpatients
- From the age of 17 you will be given a graduation clinic appointment
- You will be seen in the adult out-patient clinic
- Your appointment will be led by the adult team but a member of the children’s team (nurse or physiotherapist) will be present
- All future appointments will be held in the adult clinic
- By the age of 18 your care will be fully managed by the adult team
