Our team works closely with patients, families and carers to set meaningful goals, support recovery and enable a safe return home.
You will be under the care of a GP and supported by a multidisciplinary team including nurses, physiotherapists, occupational therapists, and support staff.
You will be encouraged to safely do as much as possible for yourself, including washing, dressing, and moving around. We will support you where needed. Our aim is to help you become as independent as possible before you return home.
How long will I stay?
Your stay will focus on supporting your recovery and is usually short term. The length of your stay will depend on your individual needs and progress but may be up to 6 weeks. We will work with you to help you return home safely as soon as possible.
What to expect?
When you arrive, the medical, nursing and therapy teams will work with you to agree a plan for your care and recovery.
We will use everyday activities alongside exercise programmes to help you achieve your goals.
To get the most from your stay
Being active will help improve your strength, confidence and independence.
We encourage you to:
- Take part in therapy sessions and daily activities
- Practise the exercises recommended by your therapy team
- Spend more time out of bed, such as sitting in your chair or walking around the ward.
- Get dressed each day where possible
We encourage you to wear your own clothes during the day to help with your recovery. Wearing your own clothes can help you stay independent, keep up everyday skills, stay active and mobile, build confidence, and prepare for a safe return home.
We also encourage relatives and carers to support and get involved.
What should I bring?
Please bring:
- Comfortable day clothes and supportive footwear
- Night clothes and well-fitting slippers
- Underwear and socks
- Bag for dirty laundry
- Toiletries
- Glasses, hearing aids, dentures and mobility aids (if you use them)
- A list of your current medications (if available)
Your belongings
You will have a bedside cupboard to store your belongings during your stay. As most cupboards do not lock, we do not recommend storing valuable items in them. We do not accept liability for loss or damage to your property unless you hand it in for safekeeping and receive a receipt. Please speak to the ward staff for more information
Medications
- Please bring any prescription medications from home with you.
- We will review your medications during your stay.
- We will explain any changes and make sure you understand them before you leave.
Visiting
Discharge planning starts on admission
We plan your discharge as soon as you arrive. This includes working with you and your family, carers or support network.
We will:
- Assess your mobility and daily living needs
- Arrange equipment or support at home if required
- Involve your family, carers or community services where appropriate
Before you leave, we will make sure you have a clear plan, understand your care and any support services which are put in place, and know who to contact if you need help.
Who to speak to if you have questions or concerns
Please speak to a member of the ward team or ask to speak to the nurse in charge or a member of your therapy team.
